Abstract

Asociación entre embarazo adolescente y complicaciones neonatales en el Hospital Provincial Docente Belén de Lambayeque, 2024 Objetivo: Se determinó la relación entre la gestación en adolescentes y las complicaciones neonatales, incluyendo prematuridad, bajo peso al nacer y puntuaciones bajas en el índice de Apgar. Metodología: Se trató de un estudio analítico de casos y controles, con un enfoque cuantitativo, basado en historias clínicas de gestantes adolescentes (10-19 años) y adultas (20-40 años) atendidas en el Hospital Provincial Docente Belén de Lambayeque durante el año 2024. Para la recolección de datos, se empleó una ficha estructurada, en la que se registrarán variables maternas y neonatales. El análisis estadístico se realizó mediante el cálculo de Odds Ratio (OR) permitiendo evaluar la magnitud de la asociación. Resultados: Indicaron que las adolescentes presentaron un menor nivel educativo, con 65% alcanzando solo educación primaria, frente a un 60% de educación secundaria completa y 19.5% de educación superior en no adolescentes, eran mayoritariamente solteras (64%), en contraste con las no adolescentes, donde predominó la convivencia (71%).En cuanto a las complicaciones neonatales, el embarazo adolescente se asoció con un menor riesgo de preeclampsia (OR 0.18, IC 95%: 0.03-0.71, p = 0.012) y un menor riesgo de puntajes bajos en el Apgar al minuto (OR 0.31, IC 95%: 0.18-0.49, p < 0.001). No se encontraron asociaciones significativas con prematuridad (OR 1.91, IC 95%: 0.63-6.47, p = 0.03), bajo peso al nacer (OR 1.00, IC 95%: 1.00-1.00, p = 0.14) ni con neonatos pequeños para la edad gestacional (OR 1.62, IC 95%: 0.52-5.61, p = 0.05).

Methodology: This was an analytical case-control study with a quantitative approach, based on the medical records of adolescent (10-19 years) and adult (20-40 years) pregnant women treated at the Belén de Lambayeque Provincial Teaching Hospital during the year 2024. For data collection, a structured form was used, in which maternal and neonatal variables will be recorded. Statistical analysis was performed by calculating the Odds Ratio (OR), allowing the magnitude of the association to be evaluated. Results: They indicated that adolescents had a lower educational level, with 65% reaching only primary education, compared to 60% complete secondary education and 19.5% higher education in non-adolescents, they were mostly single (64%), in contrast to non-adolescents, where cohabitation predominated (71%). Regarding neonatal complications, teenage pregnancy was associated with a lower risk of preeclampsia (OR 0.18, 95% CI: 0.03-0.71, p = 0.012) and a lower risk of low Apgar scores at 1 minute (OR 0.31, 95% CI: 0.18-0.49, p < 0.001). No significant associations were found with prematurity (OR 1.91, 95% CI: 0.63-6.47, p = 0.03), low birth weight (OR 1.00, 95% CI: 1.00-1.00, p = 0.14) or with small for gestational age neonates (OR 1.62, 95% CI: 0.52-5.61, p = 0.05).
