| dc.description.abstract | La anemia infantil constituye un problema de salud pública de gran magnitud: la OMS estima cerca de 300 millones de niños menores de cinco años con hemoglobina baja a nivel mundial, y en el Perú (2017) el 43.6% de los menores de 3 años presentaba algún grado de anemia. Más allá del componente nutricional, el proyecto parte de la premisa de que el manejo y la evolución de la anemia infantil están estrechamente ligados al funcionamiento del entorno familiar de la madre y el niño. Una familia disfuncional —marcada por baja comunicación, escasa adaptabilidad y débil apoyo mutuo— puede limitar la adherencia al tratamiento, la vigilancia nutricional y la respuesta oportuna ante la enfermedad. En el Centro de Salud Toribia Castro Chirinos (EESS I-4, Lambayeque), el personal ha observado en el servicio de CRED a madres con problemas familiares evidentes y baja motivación, lo que motiva evaluar formalmente su funcionalidad familiar. Objetivos: • Determinar el grado de funcionalidad familiar en madres de niños con anemia atendidos en el EESS I-4 del distrito de Lambayeque. • Identificar las dimensiones de la funcionalidad familiar (adaptación, participación, crecimiento, afecto y recursos) en dichas madres. • Determinar la funcionalidad familiar según el grupo etario de los niños con anemia. • Relacionar el tipo de anemia con el grado de funcionalidad familiar. En el marco teórico y antecedentes, el estudio se sustenta en el concepto de funcionalidad familiar como el conjunto de interrelaciones que permiten a una familia resolver conflictos mediante comunicación clara, empatía y apoyo emocional, favoreciendo el bienestar de sus integrantes. Se emplea el instrumento Apgar Familiar de Smilkstein (1978), que mide cinco dimensiones —Adaptación, Participación, Crecimiento (Growth), Afecto y Recursos— y clasifica a la familia como funcional o en distintos grados de disfunción. Investigaciones previas en el Perú (Arequipa, Lima, Trujillo) reportan asociaciones diversas entre disfunción familiar, adherencia a micronutrientes y desnutrición crónica infantil, lo que respalda la pertinencia de estudiar esta relación específicamente en el contexto de la anemia. Metodología • Diseño: estudio no experimental. • Población: 500 madres de niños con anemia atendidos en el EESS I-4; muestra calculada de 218 mediante fórmula para población finita. • Criterios de inclusión: madres de niños menores de 5 años con anemia, con seguro de salud, residentes en la jurisdicción y con participación voluntaria. • Criterios de exclusión: niños con hemoglobina normal, mayores de 5 años, fuera de la jurisdicción o madres que no acepten participar. • Instrumento: cuestionario Apgar Familiar (5 ítems, escala 0–4), validado internacionalmente (alfa de Cronbach 0.84). • Análisis: procesamiento de datos en SPSS v.22, con elaboración de tablas y gráficos descriptivos. En los resultados el puntaje total del Apgar Familiar (0 a 20 puntos) permite clasificar a cada familia en cuatro categorías: función normal (17–20 puntos), disfunción leve (13–16), disfunción moderada (10–12) y disfunción severa (9 o menos). Con esta clasificación se espera describir el estado funcional predominante entre las madres estudiadas y explorar su relación con la edad del niño y el tipo de anemia diagnosticada. En la aplicación de los recursos y cronograma el proyecto se desarrolla entre enero y junio de 2021, en tres fases: planeamiento (revisión bibliográfica y diseño del proyecto), ejecución (recolección y análisis de datos) y comunicación (elaboración y presentación del informe final). Es autofinanciado por el investigador, con un presupuesto total estimado de S/ 2,529.00, distribuido en remuneración del asesor (S/ 1,000.00), bienes de escritorio (S/ 109.00) y servicios como internet, empastados, fotocopias y consultoría estadística (S/ 1,420.00). En conclusión, el Proyecto ofrece un diseño metodológicamente sólido para caracterizar la funcionalidad familiar en un grupo vulnerable —madres de niños con anemia— usando un instrumento validado y de fácil aplicación en el primer nivel de atención. Sus hallazgos podrían orientar intervenciones de salud familiar que complementen el abordaje clínico-nutricional de la anemia infantil, fortaleciendo la adherencia al tratamiento mediante el trabajo con la dinámica familiar, no solo con el niño de forma aislada | es_PE |
| dc.description.abstract | Family Functioning Among Mothers of Children with Anemia Attending the I-4 Health Facility in the District of Lambayeque — 2021 Childhood anemia constitutes a public health problem of great magnitude: the WHO estimates that approximately 300 million children under five years of age worldwide have low hemoglobin levels, and in Peru (2017), 43.6% of children under three years of age presented some degree of anemia. Beyond its nutritional component, this project rests on the premise that the management and progression of childhood anemia are closely linked to the functioning of the family environment surrounding the mother and child. A dysfunctional family—characterized by poor communication, limited adaptability, and weak mutual support—may hinder treatment adherence, nutritional monitoring, and timely response to illness. At the Toribia Castro Chirinos Health Center (I-4 Health Facility, Lambayeque), staff in the Growth and Development Monitoring (CRED) service have observed mothers with evident family problems and low motivation, which prompted a formal assessment of family functioning. Objectives: • To determine the degree of family functioning among mothers of children with anemia treated at the I-4 Health Facility in the district of Lambayeque. • To identify the dimensions of family functioning (adaptation, participation, growth, affection, and resolve) among these mothers. • To determine family functioning according to the age group of children with anemia. • To relate the type of anemia to the degree of family functioning. Within the theoretical framework and background literature, the study draws on the concept of family functioning as the set of interrelationships that enable a family to resolve conflicts through clear communication, empathy, and emotional support, thereby fostering the well-being of its members. The study employs Smilkstein's Family APGAR instrument (1978), which measures five dimensions—Adaptation, Participation, Growth, Affection, and Resolve—and classifies families as functional or as exhibiting varying degrees of dysfunction. Prior research conducted in Peru (Arequipa, Lima, Trujillo) has reported diverse associations between family dysfunction, micronutrient adherence, and chronic childhood malnutrition, supporting the relevance of examining this relationship specifically in the context of anemia. Methodology • Design: non-experimental study. • Population: 500 mothers of children with anemia treated at the I-4 Health Facility; sample size of 218 calculated using the finite population formula. • Inclusion criteria: mothers of children under 5 years of age with anemia, covered by health insurance, residing within the jurisdiction, and voluntarily participating. • Exclusion criteria: children with normal hemoglobin levels, over 5 years of age, residing outside the jurisdiction, or mothers who decline to participate. • Instrument: Family APGAR questionnaire (5 items, 0–4 scale), internationally validated (Cronbach's alpha 0.84). • Analysis: data processed in SPSS v.22, with the preparation of descriptive tables and graphs. In terms of results, the total Family APGAR score (0 to 20 points) classifies each family into four categories: normal functioning (17–20 points), mild dysfunction (13–16), moderate dysfunction (10–12), and severe dysfunction (9 or fewer). This classification is intended to describe the predominant functional status among the mothers studied and to explore its relationship with the child's age and the diagnosed type of anemia. Regarding resource allocation and the timeline, the project runs from January to June 2021, across three phases: planning (literature review and project design), execution (data collection and analysis), and dissemination (preparation and presentation of the final report). It is self-funded by the researcher, with a total estimated budget of S/ 2,529.00, allocated to advisor compensation (S/ 1,000.00), office supplies (S/ 109.00), and services such as internet, binding, photocopying, and statistical consulting (S/ 1,420.00). In conclusion, the project offers a methodologically sound design for characterizing family functioning within a vulnerable group—mothers of children with anemia—using a validated instrument that is easily applied at the primary care level. Its findings could inform family health interventions that complement the clinical-nutritional management of childhood anemia, strengthening treatment adherence by addressing family dynamics rather than focusing solely on the child in isolation. | es_PE |
